Customer service has been excellent and helped me work through this issue. It seems like I got a rare defective unit and they are sending a replacement fan at no charge. I have really appreciated how proactive and helpful they have been. Excellent service!
If the replacement works as well as my original purchase (without the hiccups that one had), Ill be updating this to a 5 star review soon.
One thing I neglected to mention in my first review is we love the look of the oak side (as you can see in my attached photo). We had originally planned to use the black side of the fan blades but we liked the gray oak side so much that we used that side.

Fan refuses to turn on and stay on without numerous attempts. Several false starts as shown in the accompanying video. So frustrating to have installed it and need to remove and return it!
If it just worked, Id give it 5 stars.
Noise level is pretty quiet on those rare occasions when it is working.
